Fax
- Read, Sign and fax back the client
agreement.
- Have the applicant read and sign the authorization
form.
- Fill out the order form and fax it to us.
- You may submit applicant information using the order
form in conjunction with your own company application or applicant's
resume if you so prefer.
- If this is your first order, we will call to confirm it. At
that time, we will get payment information.
Please note that we need to have the applicant authorization form
faxed to us if you are ordering credit reports, reference verifications,
driving records, or degree verifications. This information is not
public record and in most cases we must provide the authorization
form before information will be released.
